You can now complete a web-based version of the WPC Questionnaire.
This version provides the same level of confidentiality, but it can
be taken on-line!

IMPORTANT:
Please e-mail Gail (gail@wpcdurham.org) after completing the web-based version of the questionnaire, so we can take your name off the "still needs to complete the questionnaire" roster.
There are still printed copies of the questionnaire available in the Mission Center or by request from Gail. We really want everyone to participate in this process!
